I just flew JAL on Sunday, January 9th (BKK, Narita, LAX)

The only thing Thailand requested at check-in was proof of Covid test.

But they also give you two forms to complete while you wait in line.

One is a small paper from the USA requesting where you will stay in USA with phone number and email address.

This is due at check in in Thailand.

The other is from Japan's Health Department inquiring what your vaccination status is (2-3 questions). 

This is submitted at check in in Japan before you enter the gate area.

I have a Global Entry and did not recall any other necessary forms to enter the USA.

One quick correction -- as noted in the posts immediately above your, it is the"day before," not "24 hours before." This is a very big deal for me, as my return flight to HND doesn't leave until around 10 p.m.

 

USA only requires an Antegen, but I took the PCR because I was confused by the JAL website of what was required to enter Japan.

I found a service (MedEx) where you can to to the lab or the nurse will come to your hotel.
