You'll need a Visa one way or another. You can get a Visa upon arrival at the airport. But If you have a few days in Bangkok, you can use a travel agency to get you a expedited visa and ticket to Vietnam. I think you need about 3 days. I've done it several times with no issues. In Bangkok, I've used Thai Charmer Travel, but you can use pretty much anybody.

I don't think it's necessary to bring US currency. They'll certainly take US currency at the Airport and metropolitan areas if you have it, but you'll likely be overpaying. There are plenty of ATMs around to draw cash from.

Yes, there's a service charge for expedited service. I *think* you can get it as soon as next day, 3 days or 7 days. Different service charges apply depending on how soon you need it. Be prepared to pay about 76 Euro for the fastest service. Don't forget to bring your passport and have 4 passport photos with you to save time.
