Unusual to have a hole without sufficient barriers and the like

not really a big hole but one may surmise that , not looking where going properly , speed , and skinny tyres . the fatter tyres make a lot of difference when hitting ruts , cracks and uneven surfaces like that . 

certainly did a number on herself . hope she gets better

They've covered it in an iron sheet....death trap they are when it rains.

I think that mid range helmet probably saved her life.
