When I have used my cards there has been no charges apart from what my bank charge anyway.

I'm with a credit union, not a bank and not only is there no service charge on my end, I get any overseas ATM fees refunded upon my return. I don't remember exactly how much the service charge was though as I only used my ATM a few times, but I do remember even with the refund, the exchange rate was much less than with TCs.

I think they do...but it is still quite cheap, in percentage it is negligeable when getting cash by 5000ot 10000 bahts bunches...
