The best advise I can give you  is rent the oldest and dirtiest scooter possible . That way if a fly happens to land on your scooter you will not be charged for damages . And NEVER GIVE THEM YOUR PASSPORT . 

Get an International Driving Licence in Your own country before you travel to Thailand. Usually valid for twelve months and very cheap. Reputable bike renters will have you covered for insurance in the rental price. Wear a helmet at all times and have your International Driving at all times. Police have very regular checkpoints. Email me at: [email protected] and I will give you name of Thai Lady that I have Rented from for the last three years. She will deliver bike to where you are staying and she is as competitive with price as any of the rest of them.
