Iv only learned about this through lurking round these forums I see it mentioned quite a bit, trying to google it (from the UK) and it seems to be PEP for all results not PrEP, I would prefer to down the route of taking it everyday while I’m there, are they both the same pill? Can I order it online in the UK I’d like to start them just before I go on my trip, failing that, I can buy them there? Where? And what do I ask for? Type it into the phone and show them?

You take 200 mg a day until you leave. You will be more sensitive to UV from sunlight.

You can buy it in Pattaya at any pharmacy and I doubt it's even 10 or 15B a tablet. There's no need to buy it online for God knows how much.

Since most pharmacists speak English I'd suggest asking for it in English.

It's not a drug of choice for post exposure. There's evidence that is reduces risk to bacterial STIs (not HIV or Hepatitis B) and might reduce the risk 50%. It's an odd paradox. Syphilis is treated differently but this drug seems to have value prophylactically.

Understand what that means. If you have unprotected sex with someone with say, syphilis, the odds of getting it go from 51 to 64% to maybe 25 to 32%. Or ultimately 100% if you are exposed 4 times. Or 100% the first time. Probability works like that.

It is not 100% going to prevent bacterial STIs in other words. HIV PrEP is much more effective in terms of what you might think as "immunity", same as HIV treatment for infected persons can reduce their viral load to be practically unmeasurable.

As of my knowledge cutoff in October 2023, the use of doxycycline as post-exposure prophylaxis (PEP)—often referred to as "Doxy PEP"—has shown promising results in reducing certain bacterial sexually transmitted infections (STIs) among high-risk populations, such as men who have sex with men (MSM) and transgender women. However, its use as pre-exposure prophylaxis (PrEP) is still under investigation and not widely recommended due to concerns about antibiotic resistance and other factors.

### Efficacy Data:

- **Reduction in Overall Bacterial STIs**: In the **DoxyPEP** randomized controlled trial, participants taking a single 200 mg dose of doxycycline within 72 hours after unprotected sexual activity experienced a **66% reduction** in the incidence of bacterial STIs compared to the control group.

- **Syphilis and Chlamydia**: The study showed a significant reduction in cases of **syphilis** and **chlamydia** among those using doxycycline PEP.

  - *Syphilis Reduction*: Approximately **70% decrease** in new infections.
  - *Chlamydia Reduction*: Around **60% decrease** in new cases.

- **Gonorrhea**: The reduction in gonorrhea cases was less pronounced, likely due to existing antibiotic resistance in *Neisseria gonorrhoeae*.

  - *Gonorrhea Reduction*: About **40% decrease**, but with regional variations due to resistance patterns.

### Considerations:

- **Antibiotic Resistance**: There's a significant concern that widespread use of doxycycline for STI prevention could contribute to increased antibiotic resistance, making infections harder to treat in the long run.

- **Side Effects**: Common side effects of doxycycline include gastrointestinal discomfort, photosensitivity (increased sensitivity to sunlight), and the potential for esophageal irritation or ulcers if not taken properly.

- **Public Health Guidelines**: As of now, health organizations have not broadly recommended doxycycline PrEP due to the need for more extensive research on long-term effects, optimal dosing strategies, and the balance between benefits and risks.

### Conclusion:

Doxycycline PEP has demonstrated effectiveness in reducing certain bacterial STIs among high-risk groups when taken shortly after unprotected sex. Specifically, studies indicate:

- **66% overall reduction** in bacterial STIs.
- Significant decreases in **syphilis** and **chlamydia** infections.
- Modest reduction in **gonorrhea** cases, with effectiveness potentially limited by antibiotic resistance.

However, its use as a preventive measure before exposure (PrEP) is not yet established practice. Ongoing research aims to better understand the implications, and individuals should consult healthcare providers for personalized advice.

**Note**: Always consult a medical professional or public health official for the most current information and personalized medical advice.

I got a couple of boxes of PrEP from the NHS last Wednesday, ready for my trip on Friday.

I’m most definitely going to be using condoms at all times, I just thought I’d take it as an extra precaution

Two things getting mixed up

1) Doxy PEP, 200mg Doxycycline, taken within 72 hours of sex. To prevent Chlamydia, Gonorrhea, Syphilis. I've not caught any of these the last year I've used it. Less than 50 baht for 5x200mg

2) PrEP, to prevent HIV, i take daily, can take on demand, before and after sex. In Pattaya can get from Pulse Clinic but available elsewhere cheaper but more hassle

If you are going to use condoms then pointless taking unnecessary drugs

So it doesn’t help reduce the risk of the skin to skin contact based ones?

No, the skin to skin ones like genital warts and herpes aren't prevented by condoms either

You can buy at any pharmacy in Thailand. Ask for doxycycline.

They gave you doxycycline? Hard to imagine the NHS would give you that.. Or are you talking Truvada?

When talking prep, normally it's for HIV. Either Truvada or Descovy (or a cheaper generic of one of those) Doxyprep is different, as it could offer some protection against bacterial infections, but does nothing to prevent HIV or other viral infections.

I got PrEP or the NHS equivalent but as they were saying do you want 3 or 4 boxes and I was thinking don’t be greedy etc, I forgot about asking about doxycycline, I choose 4 lol

doxycycline is easy to get in England anyway it’s just more expensive then Thailand, I just wanted to know what they thought about it.

i think last time I bought 90 tabs for about £20 in the UK from Dr Fox
