I can't speak for the Northwest/Delta/etc, but USAir for such such a change charges about $375. At least as of last week when I did it on a Star Alliance reservation I had (biz class-international).

Yes. I think that and any change is charged at $50 for this award ticket, since I am a Platinum member.
