Dengue Fever Outbreak

BANGKOK: -- The Public Health Ministry has set up a national war room to control the spread of dengue fever across the country after learning the situation will be far worse than last year.

So far, 1,675 people have fallen victim to the fever this year, said the Minister, Witthaya Kaewparadai.

The ministry's deputy permanent secretary, Dr Paijit Warachit will chair the war room - teaming up with agencies including the Bangkok Metropolitan Administration's Health Department, the World Health Organisation, and a joint activity of the Ministry of Public Health of Thailand and the US Center for Disease Control and Prevention, to monitor the spread of disease within Thailand and neighbouring countries.

They will meet at least once a week to keep a close watch on any possible outbreak.

Wittaya added he has assigned all provincial public health offices, local administration organisations and health volunteers to strictly control and monitor the transmission of dengue fever in local areas.

Local authorities have been advised to spray insecticide to eradicate the adult aedes aegypti mosquito, which bites humans in the daytime.

Residents should drain off any stagnant water to prevent mosquitoes from breeding and use abates sand, a pesticide, to destroy their larvae.

Citing Bureau of Epidemiology records, Paijit said in January this year, 1675 people had been infected with dengue fever and two had died. The figure for the same period last year was 1,553 dengue fever cases reported and two lives lost.

The Central region was the hardest hit with 886 patients, the Southern region suffered the second worst outbreak with 546 patients and Bangkok, 303. Songkhla, Pattani, and Yala were the worst affected provinces. Nearly half the patients this year were over 15 years old.

Last year saw a total of 91,003 patients, with 65,581 cases in 2007.

In a bid for early detection of dengue fever infection, Wittaya said he has asked the Department of Medical Services to study a test kit to screen patients.

This month the Department of Disease Control will organise meetings to brainstorm on prevention and control planning in 30 high risk areas nation-wide.

Wittaya has asked the public to keep a close watch on people in their households and nurseries. If anyone develops a high fever over a two-day period, they should be taken to a doctor.

The aedes aegypti mosquito is the transmission agent for

Dengue Fever. It bites humans between an hour or so after

dawn until about an hour after sunset.

Is there a shot or vaccine for this that I should get before coming to Pattaya, or is it the nets and fog and try not to get bit?

Cheers,

Mike

Really shitty (aka break-bone fever) rather than fatal most of the time, though it can kill. Fatalities tend to be the very young and the very old. There is no vaccine yet so avoid being bitten by mosquitoes if possible.

Dengue Fever is serious you really dont want to get it I know a well known Gogo owner that contracted it in Pattaya a few years back he told me it took him about 6 months to fully recover and yes it can be fatal.

There are several vaccines for Dengue Fever currently undergoing trails whether they prove to be effective, safe or get approval is however another matter altogether.

The best line of defense is prevention try to avoid getting bitten especially in the daytime (it's the daytime ones that carry Dengue) although you are more likely to get bitten in the evening or at night best bet is to wear an all over condom use a mosquito repellant containing DEET.

After saying all that I really wouldn't worry to much although they are most definatly here mosquitos are not that much of a problem in Pattaya although a quick squirt of mosquito repellant before you go out is not a bad idea.

I was told, the quicker you get treatment, the faster it can be shaken off!

Dengue Fever is a virus there is no effective cure treatment is mainly confined to treating the symptoms of the fever reducing pain and reducing body temperature.

If you suspect you may have it the sooner you get medical treatment the better you will feel very ill.

There are four different subtypes of Dengue Fever. Infection to one subtype will provide immunity but only to that particular subtype not to other subtypes.

Which means it's possible for someone to have more than one dengue infection during their lifetime.

I had it a couple of years ago (in Phuket). I can tell you that I do not EVER want it again and nobody in their right mind wants to risk catching it. I think the version I had was haemorrhagic fever - chills and fever, aching bones, abject lethargy, headaches and, for some strange reason, once the worst of the symptoms had subsided, all the skin flaked off my arms and legs as though I had had a really bad sunburn. Take precautions and avoid it.

I also got Dengue fever 2 years ago IN PATTAYA!

All the symptoms above as well as a few weeks of complete body spots. Feeling of impending doom all the time and every other symptom in every part of the body....an absolute nightmare. Took 6 months to fully recover and now here i sit 15kg lighter ....best bloody diet regime i ever had.

1 point is that although there is nothing the hospital can do (except re-hydrate with a drip), they sometimes have to treat complictions such as bowel infections etc which have derived from the Dengue infection.

Oh and dont go to soi 4 Pattaya International Hospital (unless you have good insurance to pick up the bill)

Just a request--for those of you unfortunate to have got it--you have my deepest sympathies. I hope I never get it---

but could you post some of the particulars about where and how you think it happened?

For example--on the ground floor of a hotel. A hotel with a fountain or pool, room was located near xxx

Or perhaps you were out till dawn, or went for a early breakfast near a drainage ditch...

Also, anything you do/would do differently to prevent/avoid.

Again my sympathies to those who have had such a horrible experience.

There are four types of dengue viruses that cause dengue fever worldwide - Dengue 1, 2, 3 and 4. A person infected with one type of dengue will subsequently only be immune to that type. They will not be immune to other types of dengue and will, in fact, be at risk of developing severe symptoms if they contract another type of dengue. I have now had 3 of the 4 types

Dengue fever symptoms:

Typical symptoms may include:

sudden onset of fever (lasting three to seven days)

intense headache (especially behind the eyes)

muscle and joint pain (ankles, knees and elbows)

unpleasant metallic taste in mouth, loss of appetite, vomiting, diarrhoea, abdominal pain

flushed skin on face and neck, fine skin rash as fever subsides

rash on arms and legs, severe itching, peeling of skin and hair loss (The rash tends to be restricted to the arms and legs)

minor bleeding (nose or gums) and heavy menstrual periods for women

extreme fatigue

It tends to be the Asian Tiger Mosquito that carries dengue and it is called that due to its prominent stripes

strped_mossie.jpg

This type of Mosquito is usually found in high vegetation areas, There is no treatment as such, prevention is by far the best, wear insect repellent, use a Mossie net if up country. Otherwise;

See a doctor immediately.

Wear insect repellent. (If you have dengue fever, mosquitoes that bite you may pass dengue on to other people even though they are not striped Mossies).

Drink plenty of liquids and take paracetamol for fever and pain. Do not take aspirin because it can affect blood clotting.

I handled types 1 & 4 reasonably well, I was hospitalized with type 2 and am awaiting with interest Type 3. I contracted all of them upcountry.

hope that helps

Signs and symptoms....

This is manifested by a sudden onset of severe headache, muscle and joint pains (myalgias and arthralgias—severe pain gives it the name break-bone fever or bonecrusher disease), fever, and rash. The dengue rash is characteristically bright red petechiae and usually appears first on the lower limbs and the chest; in some patients, it spreads to cover most of the body. There may also be gastritis with some combination of associated abdominal pain, nausea, vomiting, or diarrhea.

Some cases develop much milder symptoms which can be misdiagnosed as influenza or other viral infection when no rash is present. Thus travelers from tropical areas may pass on dengue in their home countries inadvertently, having not been properly diagnosed at the height of their illness. Patients with dengue can pass on the infection only through mosquitoes or blood products and only while they are still febrile.

The classic dengue fever lasts about six to seven days, with a smaller peak of fever at the trailing end of the disease (the so-called biphasic pattern). Clinically, the platelet count will drop until the patient's temperature is normal.

Treatment......

The mainstay of treatment is timely supportive therapy to tackle shock due to haemoconcentration and bleeding. Close monitoring of vital signs in critical period (between day 2 to day 7 of fever) is vital. Increased oral fluid intake is recommended to prevent dehydration. Supplementation with intravenous fluids may be necessary to prevent dehydration and significant concentration of the blood if the patient is unable to maintain oral intake. A platelet transfusion is indicated in rare cases if the platelet level drops significantly (below 20,000) or if there is significant bleeding. The presence of melena may indicate internal gastrointestinal bleeding requiring platelet and/or red blood cell transfusion.

Aspirin and non-steroidal anti-inflammatory drugs should be avoided as these drugs may worsen the bleeding tendency associated with some of these infections. Patients may receive paracetamol preparations to deal with these symptoms if dengue is suspected.

Hope this helps..

Gyro
